How to Apply
To be eligible for the Graeme Clark Cochlear Scholarship, submit the application (download application PDF) and following information to:

Selecting the Winners
After August 1, 2009 a panel of judges will review all submitted scholarship applications. Announcement of the winners will be in December 2009.
Please submit the following information:
1. The application form signed in ink.
2. Final school academic transcript (attach copy).
3. The applicant’s university admissions notice (attach copy).
4. A succinct list of the principal activities and awards in school (including prizes, scholarships, offices held, athletic record, extracurricular interests and substantial activities) with dates. These must not exceed one page in length, and should be set in a typeface no smaller than 10 points.
5. A short personal statement describing the applicant’s academic aspiration and other interests. This should describe why the applicant chose the proposed area of study (major), the applicant’s aspirations post-graduation, definition of success and the reasons why they wish to be a recipient of the Graeme Clark Cochlear Scholarship. This statement must not exceed 1,000 words and should be written in as simple and direct a manner as possible. The applicant must sign the statement in ink.
6. One photocopy of the applicant’s birth certificate, passport or other proof of age and citizenship.
7. The letters of three references. (At least two of these must be from someone who the applicant has received education instruction.)
8. Written proof of residence (photo copy of driver's license).
