And now, Service Agreements can now be purchased for any eligible external ESPrit™3G or Freedom™ system under original warranty that will expire on or before December 31, 2009*. In order to take advantage of this offer you must contact Customer Service at serviceagreement@cochlear.com and your payment must be received on or before June 30, 2009.

How do I get started?

When does the service agreement go into effect?
Your coverage begins the first day following the expiration of the original warranty.

**Offer is valid on Service Agreement Plans A, B, and C only. Offer is not valid on Advance Processor Purchase Plan. No Service Agreement will go into effect until Cochlear receives a completed and signed Service Agreement application form with full payment. Payment and application form must be received by Cochlear no later than June 30, 2009. You may not elect monthly payments if you wish to take advantage of this offer. If your sound processor is currently under the original 3-year warranty, it must expire on or before December 31, 2009 to take advantage of this offer. The initial term of the Service Agreement is 12 months. Following the initial term, the agreement will automatically renew for successive 12-month terms at the newly established annual fee. The Service Agreement cannot be canceled during the initial 12-month term. Following the initial 12-month term, the agreement is only cancelable if the recipient provides written notice not to renew at least 30 days in advance of term expiration. Cochlear will not refund amounts paid toward Service Agreements. Offer is valid on direct-to-recipient sales only. Offer may not be used with any other offers, contracts, promotions, or credits. Offer is available within the United States only. For offer details in other countries please contact customer service or your local Cochlear representative office. Service Agreement pricing is available in the U.S. and Canada only, and is subject to applicable taxes and additional terms and conditions.
